A 2 x 2 inch color photograph taken in the last 6 months is required.If your ID was issued less than a year ago, you MUST bring a Department of State Approved Secondary Identification (Previously issued ID even if it is expired is acceptable)įor non-New York, New Jersey or Connecticut residents, you MUST bring your driver’s license or State ID and a Department of State Approved Secondary Identification Note: you may be asked to present an additional ID when presenting one of these three documents. Other documents: 1) In-state, fully-valid learner’s permit with photo, 2) In-state, fully-valid non-driver ID with photo, or 3) temporary driver’s license with photo.Enhanced Tribal Cards and Native American tribal photo IDs.Trusted Traveler IDs (including valid Global Entry, FAST, SENTRI, and NEXUS cards).Permanent Resident Card (Green Card) – commonly used by a parent of a U.S. Matricula Consular (Mexican Consular ID) – commonly used by a parent of a U.S.Government employee ID (city, county, state, or federal).In-state, fully valid driver’s license or enhanced driver’s license with photo.Proof of Identity (if applicable, bring along one b/w or color copy of the secondary ID).Original birth certificate or a certified copy (no hospital certificates or photocopies).Proof of Citizenship (one of the following along with one b/w or color copy).The Library has copy machines (additional fees apply) and can assist you with making copies. Note: Copies of ID MUST contain the front and back of the ID on one page. DO NOT use any whiteout on the form. DO NOT sign the DS-11 form until the Passport Agent instructs you to do so. If filling out by hand, please print legibly and use black ink. Take your printed receipt listing your confirmation number and appointment details.Notes: If printing, print single-sided and bring all pages including instructions with you to the appointment.Review and confirm your information, then select “Schedule Appointment” to finalize. ![]()
